Please call us after completing this form. We need to receive a deposit of $100 towards the cost of your ad placement in order to begin our design work. The deposit is applied as a credit towards the cost of your ad placement. Please note Creating Community maintains the copyright on all design work. Ask about our affordable options for purchasing rights to use the design in other media.

Once we create your design, you'll receive a proof via email and then have the opportunity to request changes or corrections.
